Has any language implemented genericity using class substitution as
described in the book "Object Oriented Type Systems" by Palsberg and
Schwartzbach?
If so was this reasonably efficient?
Is the workbench described in Appendix B still available?

I don't know this book, so if you can described what this implementation method is about, it may help.
